Summary
Meloxicam can reduce the blood pressure lowering effect of valsartan and hydrochlorothiazide. Together with an ARB and a diuretic it increases the risk of acute kidney injury and hyperkalemia, particularly in older or volume-depleted patients.
What this interaction means in plain words
Hydrochlorothiazide-valsartan and Meloxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-valsartan belongs to Cardiovascular; Meloxicam belongs to Musculoskeletal.
Why it happens
Inhibition of renal prostaglandin synthesis causes sodium and water retention and reduced glomerular perfusion, which counteracts the antihypertensive effect and impairs renal function when combined with ARB and diuretic.
What to do
Use the lowest effective NSAID dose for the shortest time and keep the patient hydrated. Monitor blood pressure, serum creatinine and potassium, especially at initiation.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
