Summary
Both hydrochlorothiazide and salmeterol can lower serum potassium. Combined use, particularly with high-dose salmeterol or in patients with hypoxia, may produce clinically significant hypokalemia and raise arrhythmia risk. Valsartan partly offsets potassium loss.
What this interaction means in plain words
Hydrochlorothiazide-valsartan and Salmeterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-valsartan belongs to Cardiovascular; Salmeterol belongs to Respiratory.
Why it happens
Additive hypokalemia from thiazide-induced renal potassium wasting and beta-2 agonist-driven intracellular potassium shift.
What to do
Monitor serum potassium in patients with risk factors such as heart disease, hypoxia or high-dose beta-agonist use. Replace potassium if levels are low.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
