Summary
Phenobarbital adds to the CNS and respiratory depressant effects of opioids, risking profound sedation, coma and death. As a CYP3A4 inducer, it may also lower hydrocodone levels over time, and exposure may rise sharply when phenobarbital is stopped.
What this interaction means in plain words
Hydrocodone and Phenobarbital can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive CNS depression, plus phenobarbital induction of CYP3A4 affecting hydrocodone clearance.
What to do
Avoid or limit to the lowest doses and shortest duration, monitoring sedation and respiratory rate. Reassess hydrocodone dosing when phenobarbital is started or discontinued.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
