Summary
Naltrexone is an opioid antagonist and blocks the analgesic effect of hydrocodone, and can precipitate withdrawal in opioid-dependent patients. Bupropion also inhibits CYP2D6 and may reduce conversion of hydrocodone to hydromorphone, and the combination carries seizure and serotonergic concerns.
What this interaction means in plain words
Bupropion-naltrexone and Hydrocodone are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Bupropion-naltrexone belongs to Miscellaneous; Hydrocodone belongs to Musculoskeletal.
Why it happens
Naltrexone competitively antagonizes opioid receptors; bupropion inhibits CYP2D6 and lowers seizure threshold.
What to do
Avoid combining; the bupropion-naltrexone product is contraindicated with chronic opioid use. Allow an opioid-free interval of 7 to 10 days before starting it.
