Summary
Some oral mesalamine products use pH-dependent coatings to release drug in the distal gut. Famotidine raises gastric pH, which could cause premature release from certain formulations and alter delivery. Separately, NSAIDs such as ibuprofen have been linked to renal injury, which is also a recognized rare effect of mesalamine.
What this interaction means in plain words
Ibuprofen / famotidine and Mesalamine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Famotidine-related increase in gastric pH may alter release of pH-dependent mesalamine formulations, and both mesalamine and NSAIDs can be nephrotoxic.
What to do
Monitor renal function periodically and review colitis symptoms; consider a non-pH-dependent mesalamine formulation if acid suppression is long term.
