Summary

Famotidine and other H2 blockers are renally secreted by cationic transporters and may modestly raise metformin levels. NSAIDs such as ibuprofen can impair renal function, which can raise metformin exposure and the risk of lactic acidosis in susceptible patients.

What this interaction means in plain words

Ibuprofen / famotidine and Saxagliptin-metformin have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Competition for renal tubular cation transport and NSAID-related reduction in renal perfusion decrease metformin clearance.

What to do

Monitor renal function in at-risk patients, such as the elderly or those with kidney disease, and limit NSAID duration. Reduce the metformin dose if renal function declines.
