Summary

Rifabutin is a CYP3A4 inducer and may speed up the conversion of ifosfamide into both its active alkylating metabolites and its toxic by-product chloroacetaldehyde. This can raise the risk of neurotoxicity, nephrotoxicity and myelosuppression while the efficacy impact is uncertain.

What this interaction means in plain words

Ifosfamide and Rifabutin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Ifosfamide belongs to Oncology; Rifabutin belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Induction of CYP3A4 (and CYP2B6) by rifabutin increases ifosfamide bioactivation and N-dechloroethylation to neurotoxic and nephrotoxic metabolites.

What to do

Monitor for encephalopathy, renal function and blood counts during ifosfamide cycles, and consider an alternative antimycobacterial that is not an enzyme inducer if feasible.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
