Summary

Parecoxib is converted to valdecoxib, which is cleared partly by CYP2C9 and CYP3A4. Imatinib inhibits CYP2C9 only weakly and CYP3A4 moderately, so a modest increase in valdecoxib exposure is possible. Separately, NSAIDs can increase gastrointestinal bleeding and fluid retention, which are also concerns with imatinib.

What this interaction means in plain words

Imatinib and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Imatinib inhibits CYP3A4 and CYP2C9, potentially reducing valdecoxib clearance, alongside overlapping toxicities.

What to do

Use the lowest effective parecoxib dose for the shortest time. Monitor for gastrointestinal bleeding, edema and renal function.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
