Summary

Lonafarnib is a sensitive CYP3A4 substrate, and imatinib inhibits CYP3A4. Increased lonafarnib exposure can worsen gastrointestinal toxicity, hepatic effects, and QT-related risks. Labeling contraindicates strong or moderate CYP3A inhibitors.

What this interaction means in plain words

Imatinib and Lonafarnib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Imatinib belongs to Oncology; Lonafarnib belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Imatinib inhibits CYP3A4 metabolism of lonafarnib, raising its systemic exposure.

What to do

Avoid the combination. If a CYP3A inhibitor is essential, follow labeling for temporary lonafarnib dose reduction and monitor for toxicity.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
