Summary

Imatinib inhibits CYP3A4 and CYP2D6, both involved in pimozide clearance, and may raise pimozide concentrations. Imatinib can also prolong the QT interval, so the risk of dose-related QT prolongation and torsades de pointes is increased.

What this interaction means in plain words

Imatinib and Pimozide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Imatinib belongs to Oncology; Pimozide belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Imatinib inhibition of CYP3A4 and CYP2D6 reduces pimozide metabolism, combined with additive QT effects.

What to do

Avoid the combination if possible, or reduce the pimozide dose and obtain a baseline and follow-up ECG. Correct potassium and magnesium abnormalities and monitor for arrhythmia symptoms.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
