Summary
Both agents cause sedation, and lofexidine can cause hypotension, bradycardia and QT prolongation, which imipramine may add to. Tricyclic antidepressants may also blunt the antihypertensive effect of alpha-2 agonists such as clonidine, and a similar reduction in lofexidine effect is plausible.
What this interaction means in plain words
Imipramine and Lofexidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Imipramine belongs to Central Nervous System; Lofexidine belongs to Central Nervous System.
Why it happens
Additive CNS depression and QT effects; tricyclics may antagonize alpha-2 agonist activity, though the mechanism for lofexidine is not firmly established.
What to do
Monitor blood pressure, heart rate, sedation and ECG during co-administration. Watch for reduced withdrawal symptom control and adjust therapy accordingly.
