Drug interactions
imipramine + pantoprazole
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Chronic PPI therapy can cause hypomagnesemia, which may potentiate imipramine-associated QT prolongation and torsades de pointes risk.
Mechanism of action
Pantoprazole may cause hypomagnesemia (reduced intestinal Mg absorption) → electrolyte disturbance that increases susceptibility to QT prolongation with imipramine (a QT-prolonging TCA).
Clinical management
Use lowest effective PPI dose/duration. Check baseline and periodic serum magnesium if long-term PPI use or risk factors. Correct Mg/K as needed. Consider ECG monitoring if cardiac risk or high TCA dose; consider alternative acid suppression if recurrent low Mg.
