Summary

Quetiapine can prolong the QT interval, and indapamide can cause hypokalemia and hypomagnesemia, which further increases the risk of torsades de pointes. The risk is greatest with overdose, structural heart disease, bradycardia or other QT-prolonging drugs.

What this interaction means in plain words

Indapamide and Quetiapine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Indapamide belongs to Cardiovascular; Quetiapine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive QT risk: quetiapine blocks cardiac repolarizing potassium channels, while diuretic-induced electrolyte depletion lowers repolarization reserve.

What to do

Check and correct potassium and magnesium before and during therapy, and obtain a baseline ECG in patients with other risk factors. Consider an alternative antihypertensive if QT prolongation develops.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
