Summary
Serotonergic antidepressants impair platelet serotonin uptake and can increase bleeding risk, particularly upper GI bleeding when combined with NSAIDs. Nefazodone has weaker serotonin reuptake effects than SSRIs, so the risk is probably lower but still plausible.
What this interaction means in plain words
Indomethacin and Nefazodone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive impairment of platelet function from serotonin reuptake inhibition and NSAID cyclooxygenase inhibition, with NSAID gastric mucosal injury.
What to do
Monitor for GI bleeding and consider gastroprotection with a proton pump inhibitor in high-risk patients. Prefer an alternative analgesic where possible.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
