Summary

In preterm infants indomethacin reduces renal blood flow and can raise levels of renally cleared aminoglycosides. Oral neomycin is poorly absorbed, so the effect is generally limited unless absorption is increased by gut injury or the neonate is immature.

What this interaction means in plain words

Indomethacin and Neomycin po have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Indomethacin-induced reduction in renal perfusion and glomerular filtration decreases aminoglycoside clearance.

What to do

In neonates with substantial neomycin absorption or renal impairment, monitor renal function and consider aminoglycoside levels. No action is generally needed in other patients.
