Summary
Indomethacin can blunt the blood pressure lowering effect of telmisartan and, together with an angiotensin receptor blocker, may impair kidney function, particularly in elderly, dehydrated or renally impaired patients. Hyperkalemia may also develop.
What this interaction means in plain words
Indomethacin and Telmisartan-amlodipine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Indomethacin belongs to Musculoskeletal; Telmisartan-amlodipine belongs to Cardiovascular.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal blood flow and sodium excretion, and combined with angiotensin II receptor blockade impairs glomerular filtration and potassium handling.
What to do
Prefer an alternative analgesic such as paracetamol where possible; if the NSAID is needed, use the lowest dose for the shortest time and ensure hydration. Monitor blood pressure, serum creatinine and potassium.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
