Summary
Nonselective beta blockers such as nadolol can mask adrenergic warning signs of hypoglycemia, such as tremor and palpitations, and delay glucose recovery. They may also impair insulin secretion and glucose tolerance. Insulin-induced hypoglycemia can trigger a hypertensive response under unopposed alpha stimulation.
What this interaction means in plain words
Insulin aspart and Nadolol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Insulin aspart belongs to Endocrine; Nadolol belongs to Cardiovascular.
Why it happens
Beta-adrenergic blockade impairs glycogenolysis and counter-regulation and masks sympathetic hypoglycemia symptoms.
What to do
Monitor blood glucose more often and teach the patient that sweating may be the main warning sign. Consider a cardioselective beta blocker if clinically appropriate.
