Summary
Systemic timolol, like other nonselective beta-blockers, can mask hypoglycemia symptoms and delay glucose recovery after insulin-induced hypoglycemia. It may also modestly impair glucose tolerance. Ophthalmic timolol has lower but non-zero systemic exposure.
What this interaction means in plain words
Insulin glargine-lixisenatide and Timolol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Insulin glargine-lixisenatide belongs to Endocrine; Timolol belongs to Cardiovascular.
Why it happens
Nonselective beta-blockade blunts adrenergic hypoglycemia symptoms and glycogenolysis, and reduces insulin secretion.
What to do
Monitor blood glucose more often and counsel the patient that warning symptoms may be masked. Consider a cardioselective agent if a beta-blocker is needed systemically.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
