Summary

Fluoroquinolones, including levofloxacin, can cause both hypoglycemia and hyperglycemia in patients on insulin. Severe and occasionally fatal hypoglycemia has been reported, mainly in older patients and those with renal impairment.

What this interaction means in plain words

Insulin nph and Levofloxacin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Fluoroquinolones alter pancreatic beta-cell insulin release, producing additive glucose-lowering effects and dysglycemia.

What to do

Monitor blood glucose closely when starting or stopping levofloxacin and adjust insulin as needed. Teach the patient to recognize hypoglycemia symptoms, and if a reaction occurs stop levofloxacin and switch to a non-fluoroquinolone.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
