Summary
Salbutamol shifts potassium into cells and thiazide diuretics waste potassium in urine, so together they can cause clinically relevant hypokalemia. This is most likely with high or nebulized salbutamol doses, in acute asthma or COPD exacerbations, and may promote arrhythmias.
What this interaction means in plain words
Ipratropium-salbutamol and Methyclothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Beta-2 agonist-driven intracellular potassium shift adds to thiazide-induced renal potassium loss.
What to do
Check serum potassium and magnesium when salbutamol is used at high doses or repeatedly with a thiazide. Replace potassium as needed, especially in patients with heart disease or on digoxin.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
