Summary

Thiothixene has anticholinergic properties that may add to those of inhaled ipratropium, though systemic ipratropium exposure is low. Phenothiazine-like antipsychotics may also cause QT prolongation that can be additive with the beta-agonist effects of salbutamol.

What this interaction means in plain words

Ipratropium-salbutamol and Thiothixene can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive anticholinergic effects, plus additive QT-prolonging and hypokalemic effects of salbutamol with the antipsychotic.

What to do

Monitor for anticholinergic effects such as dry mouth, urinary retention and constipation. Check ECG and potassium if high-dose salbutamol is used in patients with cardiac risk factors.
