Summary
Non-acetylated salicylates can blunt the blood pressure lowering effect of irbesartan and may reduce renal function. The risk is greatest in older adults, in volume-depleted patients and in those with existing kidney disease. Potassium may also rise.
What this interaction means in plain words
Irbesartan and Salicylates can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of vasodilatory renal prostaglandins reduces glomerular perfusion and sodium excretion, opposing angiotensin receptor blockade and adding to renal hemodynamic stress.
What to do
Monitor blood pressure, serum creatinine and potassium when starting or changing salicylate doses, and keep patients well hydrated. Prefer the lowest effective salicylate dose or an alternative analgesic such as paracetamol if renal function declines.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
