Summary
Sparsentan is itself a dual endothelin and angiotensin II receptor antagonist, so adding an ARB produces excessive blockade of the angiotensin pathway. This raises the risk of hypotension, syncope, hyperkalemia and deterioration of renal function, including acute kidney injury.
What this interaction means in plain words
Irbesartan and Sparsentan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Irbesartan belongs to Cardiovascular; Sparsentan belongs to Genitourinary.
Why it happens
Additive pharmacodynamic angiotensin II type 1 receptor blockade, since sparsentan already has ARB activity.
What to do
Do not combine; stop the ARB before starting sparsentan. Sparsentan labeling contraindicates concomitant ARBs, ACE inhibitors and aliskiren.
