Summary
Volatile anaesthetics such as isoflurane can prolong the QT interval and sensitise the myocardium to catecholamines. Systemic beta-agonist exposure from salbutamol may add to arrhythmia risk during anaesthesia, particularly with hypokalaemia.
What this interaction means in plain words
Isoflurane and Salbutamol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Isoflurane belongs to Central Nervous System; Salbutamol belongs to Respiratory.
Why it happens
Additive QT prolongation and myocardial catecholamine sensitisation, with beta-agonist-induced hypokalaemia.
What to do
Check serum potassium before anaesthesia in patients using high-dose salbutamol and monitor ECG intraoperatively. Correct hypokalaemia and avoid giving additional sympathomimetics unless needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
