Summary
Voclosporin is a sensitive CYP3A4 substrate, and isoniazid has been reported to inhibit CYP3A4 to a modest degree. Higher voclosporin exposure could increase nephrotoxicity, hyperkalemia, hypertension and over-immunosuppression. The contraindication in labeling applies to strong CYP3A4 inhibitors, and isoniazid is a weak inhibitor, so the legacy severity looks overstated.
What this interaction means in plain words
Isoniazid and Voclosporin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Isoniazid belongs to Anti Bacterial; Voclosporin belongs to Miscellaneous.
Why it happens
Isoniazid weakly inhibits CYP3A4, reducing the metabolism of voclosporin.
What to do
Monitor eGFR, potassium and blood pressure when starting or stopping isoniazid, and reduce the voclosporin dose per labeling if renal function or tolerability worsens.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
