Summary
Isoproterenol, a beta-agonist, lowers serum potassium by shifting it into cells, and this may add to the potassium-lowering effect of hydrochlorothiazide in the combination product. Telmisartan may partly offset this. Hypokalemia can raise arrhythmia risk, and isoproterenol also may cause hypotension or tachycardia on top of the antihypertensives.
What this interaction means in plain words
Isoproterenol and Telmisartan-amlodipine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Isoproterenol belongs to Respiratory; Telmisartan-amlodipine belongs to Cardiovascular.
Why it happens
Beta-2 stimulated intracellular potassium shift adds to renal potassium loss; additive vasodilation and hypotension can also occur.
What to do
Monitor serum potassium, heart rhythm and blood pressure during isoproterenol therapy and correct hypokalemia.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
