What this interaction means in plain words
Ivosidenib and Methadone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Ivosidenib belongs to Oncology; Methadone belongs to Musculoskeletal.
Why it happens
Additive QT prolongation plus CYP3A4 induction by ivosidenib increasing methadone clearance.
What to do
Monitor ECG and electrolytes and correct abnormalities. Watch for withdrawal or reduced methadone effect and adjust the dose accordingly, and reassess after ivosidenib is stopped.
