Summary

Ivosidenib is a CYP3A4 inducer, so it may reduce salmeterol exposure and weaken bronchodilator effect. The clinical significance is uncertain because inhaled salmeterol has low systemic exposure, and the direction of effect differs from what is usually expected with an inhibitor. Ivosidenib also prolongs the QT interval, which adds a theoretical cardiac concern with the beta-agonist.

What this interaction means in plain words

Ivosidenib and Salmeterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Ivosidenib belongs to Oncology; Salmeterol belongs to Respiratory.

Evidence level: Low

Why it happens

Ivosidenib induces CYP3A4 and may lower salmeterol levels; additive QT effects are also possible.

What to do

Monitor asthma or COPD control and consider a rescue bronchodilator plan if efficacy declines. Obtain an ECG and check electrolytes if QT risk factors are present.

What to ask your pharmacist
