Summary
Ketoconazole raises mavacamten exposure and may cause excessive reduction in ejection fraction leading to heart failure from systolic dysfunction. Labeling contraindicates its use with strong CYP3A4 inhibitors.
What this interaction means in plain words
Ketoconazole and Mavacamten are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Ketoconazole belongs to Anti Fungals; Mavacamten belongs to Cardiovascular.
Why it happens
Inhibition of CYP3A4 and CYP2C19 reduces mavacamten metabolism.
What to do
Do not co-administer. Use an alternative antifungal and monitor echocardiographic LVEF per mavacamten labeling.
