For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Gastric pH elevation from pantoprazole markedly reduces oral ketoconazole absorption, risking antifungal failure.
Mechanism of action
Pantoprazole increases gastric pH → decreased dissolution/absorption of ketoconazole (requires acidic pH).
Clinical management
Avoid coadministration. If antifungal needed on PPI, use an alternative not dependent on gastric acidity (e.g., fluconazole) or stop/switch acid suppression. If unavoidable, give ketoconazole with acidic beverage and monitor response.
