Summary
NSAIDs can weaken the blood pressure lowering effect of angiotensin receptor blockers and, together with them, can impair renal function, especially in elderly or volume-depleted patients. Hyperkalemia may also occur.
What this interaction means in plain words
Ketoprofen and Olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ketoprofen belongs to Musculoskeletal; Olmesartan belongs to Cardiovascular.
Why it happens
NSAIDs reduce afferent arteriolar vasodilating prostaglandins while ARBs dilate the efferent arteriole, lowering glomerular filtration pressure; reduced prostaglandin-mediated renin and aldosterone effects also favor potassium retention.
What to do
Ensure adequate hydration and monitor serum creatinine, potassium and blood pressure after starting the NSAID. Avoid in dehydrated or renally impaired patients where possible.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
