Summary

Pimozide blocks dopamine D2 receptors in the brain and can counteract the antiparkinsonian benefit of levodopa. Parkinsonian symptoms may worsen, and pimozide itself can cause extrapyramidal effects in these patients.

What this interaction means in plain words

Levodopa and Pimozide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Levodopa belongs to Central Nervous System; Pimozide belongs to Central Nervous System.

Evidence level: High

Why it happens

Pharmacodynamic antagonism, as central D2 receptor blockade by pimozide opposes dopamine replacement from levodopa.

What to do

Avoid pimozide in patients with Parkinson disease treated with levodopa and choose a different agent for the indication. If an antipsychotic is unavoidable, consider quetiapine or clozapine.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
