Summary
Topiramate can cause hyperchloremic non-anion gap metabolic acidosis by lowering serum bicarbonate. This may complicate metformin therapy and make lactic acidosis harder to recognize or more likely. Topiramate may also modestly increase metformin exposure.
What this interaction means in plain words
Linagliptin-metformin and Topiramate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Linagliptin-metformin belongs to Endocrine; Topiramate belongs to Central Nervous System.
Why it happens
Carbonic anhydrase inhibition by topiramate reduces bicarbonate, producing metabolic acidosis that adds to metformin-associated acidosis risk.
What to do
Monitor serum bicarbonate and acid-base status, and review metformin if persistent acidosis develops.
