Summary

Combining an SSRI with a tricyclic antidepressant can raise serotonergic activity and increase the risk of serotonin syndrome. Sertraline may also modestly raise tricyclic exposure through CYP2D6 inhibition, which adds to adverse effects.

What this interaction means in plain words

Lofepramine and Sertraline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive serotonergic reuptake inhibition plus weak-to-moderate sertraline inhibition of CYP2D6 metabolism of the tricyclic.

What to do

Avoid the combination where possible, or start low and titrate slowly while monitoring for serotonin toxicity and anticholinergic or cardiac effects. Reduce the lofepramine dose if adverse effects emerge.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
