Summary

Both drugs depress the central nervous system, and giving them together can deepen sedation and impair breathing. Hypoventilation, airway obstruction, oxygen desaturation or apnea may occur and the sedative effect may be prolonged.

What this interaction means in plain words

Loxapine and Midazolam intranasal can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic CNS and respiratory depression from a benzodiazepine and a sedating antipsychotic.

What to do

Use the lowest effective midazolam dose and monitor respiratory rate, oxygen saturation and level of consciousness continuously. Make sure airway support is available and avoid additional CNS depressants.

What to ask your pharmacist
