Summary
Ivacaftor is a CYP3A substrate, but lumacaftor induces CYP3A, offsetting the effect of inhibitors at steady state. Nicardipine is also a weak inhibitor, so a clinically relevant rise in ivacaftor exposure is unlikely.
What this interaction means in plain words
Lumacaftor / ivacaftor and Nicardipine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Weak CYP3A4 inhibition by nicardipine is largely offset by lumacaftor-mediated CYP3A induction.
What to do
No dose adjustment is needed. Monitor for ivacaftor adverse effects and blood pressure, since lumacaftor induction may also lower nicardipine levels.
