Summary

Intravenous mannitol is an osmotic diuretic that can itself injure the kidney, and aminoglycosides are nephrotoxic and ototoxic. Inhaled tobramycin has low systemic absorption, but exposure can rise in patients with impaired renal function, so the combination may add to renal and cochlear risk.

What this interaction means in plain words

Mannitol and Tobramycin inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive nephrotoxicity from osmotic nephrosis and aminoglycoside tubular toxicity, with reduced aminoglycoside clearance if renal function declines.

What to do

Monitor serum creatinine, urine output and tobramycin levels when systemic exposure is likely. Avoid the combination in patients with pre-existing renal impairment where possible.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
