Summary
Voclosporin, a calcineurin inhibitor, causes dose-related nephrotoxicity. Mannitol can cause osmotic kidney injury and acute kidney injury, especially with volume depletion, so using them together may increase the risk of renal impairment.
What this interaction means in plain words
Mannitol and Voclosporin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Mannitol belongs to Cardiovascular; Voclosporin belongs to Miscellaneous.
Why it happens
Additive nephrotoxicity: calcineurin inhibitor afferent vasoconstriction combined with mannitol osmotic tubular injury.
What to do
Monitor eGFR, serum creatinine and fluid status closely during and after mannitol. Reduce or interrupt voclosporin per labeling if renal function falls.
What to ask your pharmacist
- Should I take them at different times of day?
