Summary

Both drugs can cause central nervous system effects. Metoclopramide may cause extrapyramidal reactions, and antidepressants that affect monoamines may add to these effects or worsen them. Combined use may increase the risk of movement disorders and neuroleptic malignant syndrome-like or serotonergic reactions.

What this interaction means in plain words

Maprotiline and Metoclopramide intranasal can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive central dopaminergic antagonism (metoclopramide) with monoaminergic modulation by maprotiline; the exact mechanism is not fully established.

What to do

Monitor for extrapyramidal symptoms, restlessness, agitation and serotonergic signs. Consider an alternative antiemetic if movement disorders occur.
