Summary

Ibutilide markedly prolongs repolarisation and carries a significant risk of torsades de pointes. Adding maprotiline, which also lengthens the QT interval, further increases the likelihood of life-threatening ventricular arrhythmia.

What this interaction means in plain words

Ibutilide and Maprotiline are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Ibutilide belongs to Cardiovascular; Maprotiline belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic prolongation of the QT interval.

What to do

Do not give ibutilide to patients taking maprotiline; consider an alternative cardioversion strategy such as electrical cardioversion. Review the need for maprotiline if rhythm control with QT-prolonging agents is planned.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
