Summary

Both drugs can lengthen the QT interval and slow cardiac conduction, so together they raise the risk of torsades de pointes and conduction disturbances. The risk is higher with renal impairment, electrolyte disturbances or underlying heart disease.

What this interaction means in plain words

Maprotiline and Procainamide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic effects on QT prolongation and cardiac conduction.

What to do

Avoid combining them and select an antidepressant with less cardiac effect if procainamide is required. If unavoidable, monitor ECG (QTc and QRS) and correct potassium and magnesium.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
