Summary
Yohimbine, the active alkaloid in yohimbe, is an alpha-2 antagonist that raises sympathetic outflow and can cause hypertension, tachycardia and anxiety. Maprotiline blocks norepinephrine reuptake, so combined use may exaggerate noradrenergic effects and produce unpredictable blood pressure changes.
What this interaction means in plain words
Maprotiline and Yohimbe can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive noradrenergic effects from alpha-2 antagonism and norepinephrine reuptake inhibition; the exact interaction is not fully characterised.
What to do
Advise patients to avoid yohimbe products while on maprotiline. If exposure has occurred, monitor blood pressure and heart rate.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
