For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Mavacamten is metabolized substantially by CYP2C19, with contributions from CYP3A4. Pioglitazone is not an established clinically meaningful CYP2C19 inhibitor, so the predicted rise in mavacamten exposure is uncertain. If exposure did rise, excessive reduction in ejection fraction and heart failure could occur.

What this interaction means in plain words

Mavacamten and Pioglitazone have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Mavacamten belongs to Cardiovascular; Pioglitazone belongs to Endocrine.

Evidence level: Low

Why it happens

Possible weak inhibition of CYP2C19 metabolism of mavacamten, which is not well established for pioglitazone.

What to do

No empirical dose change is needed, but follow the routine echocardiographic LVEF monitoring required for mavacamten. Reassess LVEF if clinical status changes after pioglitazone is started or stopped.
