Summary
Milnacipran reduces platelet serotonin uptake and meclofenamate, an NSAID, impairs platelet function and irritates the gastric mucosa. Together they raise the risk of gastrointestinal and other bleeding above that of either drug alone.
What this interaction means in plain words
Meclofenamate and Milnacipran can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive impairment of platelet aggregation (SNRI serotonin depletion plus NSAID COX inhibition) combined with NSAID mucosal injury.
What to do
Monitor for signs of GI bleeding such as melena, anemia or hematemesis, and use the lowest effective NSAID dose for the shortest time. Consider gastroprotection with a proton pump inhibitor in higher-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
