Summary
In patients with myasthenia gravis, corticosteroids can transiently worsen muscle weakness, particularly at the start of therapy. Cholinesterase inhibitors such as donepezil are used to improve neuromuscular transmission, and the combination has been associated with severe weakness, including respiratory compromise. The risk is mainly relevant in myasthenic patients.
What this interaction means in plain words
Memantine-donepezil and Triamcinolone acetonide injectable can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Corticosteroids may cause an early deterioration of neuromuscular function in myasthenia gravis that can interfere with the effect of anticholinesterase therapy; the exact mechanism is not fully established.
What to do
Check whether the patient has myasthenia gravis; if so, start the corticosteroid under close observation of muscle strength and respiratory function, ideally in a monitored setting. In patients without myasthenia gravis no special action is needed.
