Summary
Parecoxib is converted to valdecoxib, a COX-2 inhibitor that can reduce renal perfusion. Together with mesalamine, which can rarely cause interstitial nephritis, renal injury risk may rise.
What this interaction means in plain words
Mesalamine and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive nephrotoxicity from COX-2 inhibition of renal prostaglandins and mesalamine tubulointerstitial toxicity.
What to do
Monitor serum creatinine and hydration status, and use the lowest effective parecoxib dose for the shortest time. Avoid in patients with significant renal impairment.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
