Summary
Methoxyflurane releases fluoride during metabolism and can cause renal tubular toxicity. Oral neomycin is usually poorly absorbed but can reach significant systemic levels when the gut is inflamed or renal function is reduced, adding to kidney and ear toxicity.
What this interaction means in plain words
Methoxyflurane and Neomycin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive nephrotoxicity from fluoride-mediated tubular injury and aminoglycoside accumulation in the kidney, with ototoxicity when neomycin is absorbed.
What to do
Avoid combining where possible, especially in renal impairment or bowel disease. If used together, monitor renal function and hearing.
What to ask your pharmacist

- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
