Summary

Methylergonovine is a potent ergot vasoconstrictor, and combining it with sumatriptan can cause additive, prolonged arterial spasm. This may lead to myocardial or cerebral ischemia, or peripheral ischemia.

What this interaction means in plain words

Methylergonovine and Sumatriptan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Additive vasoconstriction through serotonin receptor agonism by both drugs and adrenergic and serotonergic effects of the ergot.

What to do

Do not give the two within 24 hours of each other. In postpartum hemorrhage use a non-ergot uterotonic such as oxytocin or carboprost if a triptan was recently taken.

What to ask your pharmacist
