Summary
Voclosporin is a CYP3A4 substrate with a narrow therapeutic window, and moderate or strong CYP3A4 inhibitors raise its exposure. Metronidazole has at most weak CYP3A4 inhibitory activity, so a clinically meaningful rise is unlikely but cannot be fully excluded. Higher voclosporin levels could worsen nephrotoxicity, hypertension and hyperkalemia.
What this interaction means in plain words
Metronidazole and Voclosporin have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Metronidazole belongs to Anti Parasitic; Voclosporin belongs to Miscellaneous.
Why it happens
Possible weak inhibition of CYP3A4-mediated voclosporin metabolism.
What to do
No empirical dose reduction is needed; monitor eGFR, blood pressure and potassium when starting or stopping metronidazole.
