Summary
Transdermal selegiline at antidepressant doses inhibits MAO-A and MAO-B systemically. Combining it with milnacipran carries a risk of serious serotonin syndrome and hypertensive crisis.
What this interaction means in plain words
Milnacipran and Selegiline transdermal are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Irreversible MAO inhibition raises monoamines while milnacipran blocks their reuptake.
What to do
Do not use together. Leave at least 2 weeks between stopping selegiline and starting milnacipran, and at least 5 days after stopping milnacipran before starting selegiline.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
